How often are your birds allowed to sit directly on you or the other humans in your home? Include time spent cuddling or sitting very close (For example: on your shoulder, lap, etc.)

What kind of cage do you have for your bird(s) and how often are they in it?

Please describe the setup of the inside of your cages as well as the area on top/surrounding it.

Does your parrot have access to toys and other forms of enrichment? Do toys get replaced regularly? Please explain.

Where are your parrot's favorite places to hang our during the day? What about at night?

Please describe what a typical day looks like for your bird.

What time does your bird wake up and go to bed? Are they allowed to come out of the cage as soon as they awaken?

Please list the main reasons you have requested coaching. Include things like behavioral issues, etc.

Do these issues usually happen at the same time each day? Or does it seem to occur randomly? Please provide details.

Have you noticed any patterns in the behavior or issues? For example: "the issue seems to happen when another bird is around" Include any patterns that involve behavior by humans in the home.
